Pell & Gregory Classification of 3rd Molar

Because of the difficulty in extraction of impacted 3rd, It was suggested that it is very important to make a classification for the impacted 3rd molar.

Introduction

Because of the difficulty in extraction of impacted 3rd, It was suggested that it is very important to make a classification for the impacted 3rd molar.

Usage of Classifications

  • Classifications Helps in assessing the best method to go for surgical extraction
  • Helps in determining the actual position of impacted 3rd molar
  • Communication language / Facilitates communication between Surgeons.

Name of Classifications

  1. Pell & Gregory Classification
  2. Winter Classification
  3. Asanami & Kasazaki Classification

Pell & Gregory Classification

It is catecorized by 2 Relations:-

1- Relation of tooth to the ramus of the mandible and the second molar

Class I:

When there is sufficient space between the anterior part of ramus and posterior/distal part of the second molar and the space accommodate with mesio-distal diameter of the impacted 3rd molar.
Class I

Class II:

When the space between the anterior part of ramus and posterior/distal part of the second molar is less than the mesio-distal diameter of the impacted 3rd molar.
Class II

Class III:

When the most of mesio-distal impacted 3rd molar is located within the ramus.
Class III

2- Relative depth of third molar in bone

Position A

When the highest part of the 3rd molar is on the level or above the occlusal plane.

Position A

Position B

When the highest part of the 3rd molar is below the occlusal plane, but above the cervical line of the second molar.
Position B

Position C

When the highest part of the 3rd molar is below the cervical line of the second molar.
